Home Health is a program for those who need nursing care, rehabilitation, or therapy at home. The Home Health team of registered or licensed nurses, social workers, homecare aides, and occupational, speech, or physical therapists work closely with your physician. Palliative Care enriches the lives of individuals and families experiencing life limiting illness. Our goal is to improve the quality of life for both patients and their families/caregivers, reducing burdensome hospitalizations when care can be delivered in the home. Hospice offers comprehensive support to terminally ill patients. In addition to providing pain and symptom management for the patient, hospice gives comfort and reassurance to families during uncertain life transitions. You should consider hospice care when there is a life expectancy of six months or less and you or your loved one have decided to forgo seeking curative measures. Cottage in the Meadow
